PA amendment of Birth record- Court order

PA amendment of Birth record- Court order

I will need a court order to amend my GF name on my mother's birth certificate (all Philadelphia County). I cannot find any information on what court to file the petition with, if anyone can respond that has done it themselves? Also if anyone has a template or sample petition.

Marriage records are e-file but Birth records are not. (already checked). Any help is appreciated!!

Check tapatalk. There was a very informative thread from about 10 years ago that included a great sample petition and order.

What do you mean you can't e-file a request to correct a birth record? I'm sure that you can...it's just a different court from the marriage records. Marriage record amendments are filed with the orphan's court whereas birth record amendment petitions are filed with the civil court.

Also, if your grandfather's birth record is from Italy and you are using it to correct his name on your mother's BC, make sure you have it apostilled and the translation must be notarized!

I used a lawyer in PA for all of this; I do not know if they can file in Philadelphia County, but if interested, send me a PM.
