Is the Latin word "quondam" (formerly) singular or plural?

"quondam" is formally an adverb, so, it has no singular or plural.
But sometimes, when it's referred to more people, it is abbreviated as "qqmm"

A death record reads "quondam father's name and mother's name".

Since the deceased was age 76, I assume both parents were also deceased, though "quondam father's name and quondam mother's name" would have been more definitive.

I agree. But, if the record is in Italian, it might read "dei quondam" or "delli quondam", that would mean that both were deceased (I don't know if that is the case)

No, just "filius quondam". It's a church record written in Latin.
