Can't understand two birth records with same name

i found two birth records from my granfather Antonio Di Marco but can't understand what is write (and why there are 2 of them).

can anyone help me?

the first looks had a twin brother called Nicolamaria, but they born dead?

Look in the death records between 1891 and 1895---you are likely to find one for the 1st Antonio. Renaming the next child after one's death with an important family name is very common.

Both of the twins died on 4 March 1891 at age 3 days.
