Need help with translating document

[say]

I was contacted by someone asking for information on their family from Nusco. I found this birth record which appears to be where his family line starts, for I believe he was adpoted. If someone could please help on transcribing this document I, and the family, would greatly appreciate your help. I will try to attach it, but if it does not work, please e-mail me at say5@earthlink.net and I will send it to you directly. Thank you again! Well, I tried to attach it and it will not work, so please contact me.
